Preliminary results released in 2003 from the Women\u2019s Health Initiative (WHI) clinical trial\u00a0<a class=\"body-link css-inlxvj emevuu60\" href=\"https:\/\/www.prevention.com\/health\/health-conditions\/a60638317\/hormone-replacement-therapy-menopause-whi-study-followup\/\" target=\"_blank\" rel=\"noopener\" data-vars-ga-outbound-link=\"https:\/\/www.prevention.com\/health\/health-conditions\/a60638317\/hormone-replacement-therapy-menopause-whi-study-followup\/\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"linked\" data-node-id=\"2.3\">linked<\/a>\u00a0hormone therapy used to treat symptoms of menopause to a higher risk of developing serious health conditions, including heart disease and stroke. The researchers analyzed biomarkers linked with cardiovascular health in nearly 2,700 women over a six-year period.<\/p>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"9\">The women were broken into two groups: Those who took estrogen and those who took a combination of estrogen and progesterone. The women were between the ages of 50 and 79. They provided blood samples at the start and various other times during the study.<\/p>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"10\">The researchers discovered that hormone therapy had a positive impact on certain cardiovascular biomarkers in the group that took estrogen, as well as those who took estrogen and progesterone. Specifically, levels of LDL (\u201cbad\u201d cholesterol) dropped by about 11%, while total cholesterol and insulin resistance (when your body\u2019s cells don\u2019t respond as well to the hormone insulin) also dropped.<\/p>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"11\">Levels of HDL (or \u201cgood\u201d cholesterol) increased by 13% in the estrogen-only group and 7% in the estrogen-progesterone group.<\/p>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"12\">Levels of a cholesterol molecule called lipoprotein(a) dropped by 15% and 20% in the estrogen-only and the estrogen-plus-progesterone groups. This finding surprised researchers because the molecule is believed to be mostly influenced by genetics.<br data-node-id=\"12.1\" \/><br data-node-id=\"12.2\" \/>There was one noteworthy unfavorable outcome: Triglycerides and coagulation factors, which are proteins in the blood that help form blood clots, increased, which is a potential downside of hormone therapy. For this reason, if you have a history of blood clots, it&#8217;s extra important to talk to your healthcare provider before taking hormone therapy.<\/p>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"13\">Interestingly, the study found that, with the exception of triglycerides, hormone therapy had positive effects regardless of age, which contrasts to prior findings. \u201cA lot of data suggests that the safest time to give hormone therapy is within 10 years of the menopause transition,\u201d Dr. Streicher says. Still, the study concludes that the results may be used to counsel\u00a0<em data-node-id=\"13.1\">younger<\/em>\u00a0menopausal women with bothersome symptoms, likely because broader research shows that it&#8217;s safest to start hormone therapy closer to when menopause starts. (Note: hormone therapy\u00a0<a class=\"body-link css-inlxvj emevuu60\" href=\"https:\/\/www.psu.edu\/news\/research\/story\/can-hormone-therapy-improve-heart-health-menopausal-women\" target=\"_blank\" rel=\"noopener\" data-vars-ga-outbound-link=\"https:\/\/www.psu.edu\/news\/research\/story\/can-hormone-therapy-improve-heart-health-menopausal-women\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"isn't FDA approved\" data-node-id=\"13.3\">isn&#8217;t FDA approved<\/a>\u00a0to treat the risk of coronary artery disease or stroke.)<\/p>\n<h2 id=\"why-is-hormone-therapy-in-menopause-controversial\" class=\"body-h2 css-ne8qws emevuu60\" data-node-id=\"14\"><strong data-node-id=\"14.0\">Why is hormone therapy in menopause controversial?<\/strong><\/h2>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"15\">It traces back to the early 2000s when those preliminary results were published from the WHI that linked hormone therapy to a higher risk of developing serious health conditions like heart disease and stroke. The study was actually stopped as a result and many women have avoided hormone therapy to treat symptoms of menopause since.<\/p>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"16\">But the preliminary conclusions were later debunked. Other researchers analyzed the data and\u00a0<a class=\"body-link css-inlxvj emevuu60\" href=\"https:\/\/academic.oup.com\/jcem\/article\/98\/5\/1771\/2536695\" target=\"_blank\" rel=\"noopener\" data-vars-ga-outbound-link=\"https:\/\/academic.oup.com\/jcem\/article\/98\/5\/1771\/2536695\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"discovered\" data-node-id=\"16.1\">discovered<\/a>\u00a0that the original study looked at women who were 65 and up who already had a greater risk of heart attack, stroke, blood clots, and more\u2014and that had skewed the data.<\/p>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"17\">The preliminary findings also didn\u2019t look at how old the women were when they started hormone therapy, which further muddled the results, explains\u00a0<a class=\"body-link css-inlxvj emevuu60\" href=\"https:\/\/www.drstreicher.com\/about\" target=\"_blank\" rel=\"noopener\" data-vars-ga-outbound-link=\"https:\/\/www.drstreicher.com\/about\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"Lauren Streicher, M.D.\" data-node-id=\"17.1\">Lauren Streicher, M.D.<\/a>, a clinical professor of obstetrics and gynecology at\u00a0<a class=\"body-link css-inlxvj emevuu60\" href=\"https:\/\/www.feinberg.northwestern.edu\/sites\/obgyn\/\" target=\"_blank\" rel=\"noopener\" data-vars-ga-outbound-link=\"https:\/\/www.feinberg.northwestern.edu\/sites\/obgyn\/\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"Northwestern University Feinberg School of Medicine\" data-node-id=\"17.3\">Northwestern University Feinberg School of Medicine<\/a>.<\/p>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"18\">In May 2024, a follow-up to the WHI study was released in\u00a0<a class=\"body-link css-inlxvj emevuu60\" href=\"http:\/\/doi.org\/10.1001\/jama.2024.6542\" target=\"_blank\" rel=\"noopener\" data-vars-ga-outbound-link=\"http:\/\/doi.org\/10.1001\/jama.2024.6542\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"JAMA\" data-node-id=\"18.1\"><em data-node-id=\"18.1.0\">JAMA<\/em><\/a>\u00a0with the conclusion that hormone therapy \u201cis appropriate\u201d for those with vasomotor symptoms (which could include hot flashes, for example) for interested women in early menopause without any contraindications.<\/p>\n<h2 id=\"why-might-hormone-therapy-lower-the-risk-of-cardiovascular-disease\" class=\"body-h2 css-ne8qws emevuu60\" data-node-id=\"19\"><strong data-node-id=\"19.0\">Why might hormone therapy lower the risk of cardiovascular disease?<\/strong><\/h2>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"20\">This isn\u2019t the first study to suggest that hormone therapy may lower the risk of cardiovascular disease for women in menopause. She points out that there\u2019s\u00a0<a class=\"body-link css-inlxvj emevuu60\" href=\"https:\/\/www.heart.org\/en\/news\/2023\/02\/20\/the-connection-between-menopause-and-cardiovascular-disease-risks\" target=\"_blank\" rel=\"noopener\" data-vars-ga-outbound-link=\"https:\/\/www.heart.org\/en\/news\/2023\/02\/20\/the-connection-between-menopause-and-cardiovascular-disease-risks\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"an association\" data-node-id=\"23.1\">an association<\/a>\u00a0between women who have hot flashes and cardiovascular risks. But hormone therapy helps to tamp down on hot flashes.<\/p>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"24\">\u201cIf you give women estrogen and these cardiovascular markers get better, is it because of the estrogen itself, the estrogen getting rid of the hot flashes, or both?\u201d Dr. Streicher says. \u201cWe don\u2019t know.\u201d<\/p>\n<h2 id=\"whats-the-best-way-to-take-care-of-your-heart-during-menopause\" class=\"body-h2 css-ne8qws emevuu60\" data-node-id=\"25\"><strong data-node-id=\"25.0\">What\u2019s the best way to take care of your heart during menopause?<\/strong><\/h2>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"26\">Taking hormone therapy is one option, but it\u2019s a decision to make with your doctor, says\u00a0<a class=\"body-link css-inlxvj emevuu60\" href=\"https:\/\/www.memorialcare.org\/providers\/jennifer-l-wong\" target=\"_blank\" rel=\"noopener\" data-vars-ga-outbound-link=\"https:\/\/www.memorialcare.org\/providers\/jennifer-l-wong\" data-vars-ga-ux-element=\"Hyperlink\" data-vars-ga-call-to-action=\"Jennifer Wong, M.D.,\" data-node-id=\"26.1\">Jennifer Wong, M.D.,<\/a>\u00a0cardiologist and medical director of Non-Invasive Cardiology at MemorialCare Heart and Vascular Institute at Orange Coast Medical Center in Fountain Valley, CA.<\/p>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"27\">\u201cRegardless of whether one decides to start hormone therapy, women during menopause should eat a heart healthy diet, exercise, and try to obtain good quality sleep,\u201d she says.<\/p>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"28\">Dr. Ross specifically recommends taking these steps to protect your heart health in menopause:<\/p>\n<ul class=\"css-kw9lqy emevuu60\" data-node-id=\"29\">\n<li data-node-id=\"29.0\">Eat a healthy colorful diet with whole grains and lean proteins<\/li>\n<li data-node-id=\"29.1\">Exercise for 150 minutes per week<\/li>\n<li data-node-id=\"29.2\">Try to maintain a healthy weight<\/li>\n<li data-node-id=\"29.3\">Aim for seven to eight hours of sleep a night<\/li>\n<li data-node-id=\"29.4\">Limit alcohol to no more than two to three drinks a week<\/li>\n<li data-node-id=\"29.5\">Try to stay on top of stress through relaxing activities like yoga, meditation, mindfulness, and psychotherapy<\/li>\n<li data-node-id=\"29.6\">Don\u2019t smoke or vape<\/li>\n<li data-node-id=\"29.7\">Get regular health and heart screenings<\/li>\n<\/ul>\n<p class=\"css-6wxqfj emevuu60\" data-journey-content=\"true\" data-node-id=\"30\">\u201cIt is important to keep in mind that menopause is the next chapter of a woman\u2019s life, not a phase that you go in and out of like puberty,\u201d Dr. Ross says. \u201cAll of these symptoms can disrupt the quality of life, but they can be lessened or eliminated,\u201d she says with lifestyle modifications, medications, and\/or hormone replacement therapy.
